What is First Link?

First Link is a partnership program with health care professionals and the Alzheimer Society of Toronto to ensure that individuals and families dealing with dementia receive support and information throughout the course of the disease.  First Link partners include family physicians, geriatric specialists, pharmacists, social service workers in health facilities and community agencies and staff working in family practice.

 

Alzheimer Society of Toronto and Rexall are partnering in order to offer additional education and support to your patients.  The Alzheimer Society’s First Link Program offers several services aimed to support you and your patients living with or caring for someone with dementia.

1.  Refer patients or their caregivers to the Toronto Alzheimer’s chapter for counselling, education, and information.  The First Link program referral link is posted on the store portal, Rx Tab, External Links sections.  With your patients' consent, fill out the online referral Form and an Alzheimer's Society counselor will contact them.  Or, print out the form and patients can do it themselves.

2.  They can arrange seminars on topics like caregiver stress and brain health, for your patients and/or the local community.  These seminars can be in-store if space permits or off-site in your community.

3.  For those of you with a private counseling room, they can arrange for a Counsellor to offer one-on-one counselling to your patients or their caregivers on-site.

4. Organize in-store displays and information booths where they can offer clients information on the Alzheimer Society’s Brain Health initiative. Call us at 416-322-6560 to book a display.
