What is First Link?

First Link is a partnership program with health-care professionals and the Alzheimer Society of Toronto to ensure that individuals and families dealing with dementia receive support and information throughout the course of the disease. First Link partners include family physicians, geriatric specialists, pharmacists, social service workers in health facilities and community agencies and staff working in family practice.

Getting Started

Ask your patient or their family member's permission to forward his or her name to the
Alzheimer Society of Toronto.
